Financial Policy Notice

To maintain compliance with state and federal regulations, managed care and preferred provider agreements, as well as billing and coding guidelines, we have adopted the following financial policies:

 

Our clinic has established a single fee schedule that applies to all patients for each service provided.

 

We are considered an out-of-network provider, with all insurance carriers. We do have a medical discount program, (see below).

 

For major medical policies, we will provide appropriate documentation (superbill) for you to submit to your insurance carrier for possible reimbursement.  Our policy is for payment to be rendered at time of service unless a prepayment or auto debit arrangement is made.

 

We will submit your bill to Medicare for “active spinal manipulation adjustments” on your behalf, but we do not accept assignment of Medicare claims.

 

• We are a network provider in a DMPO (Discount Medical Provider Organization) that you may join.

• CHUSA (Chiro Health USA) is a medical discount plan that allows us to discount our fees.

• CHUSA (Chiro Health USA) Membership is $49.00 annually and covers you and your dependents.
